To make a referral for Residential, Day Program, and Intensive Outpatient Services we request that the following documents are forwarded to our Admissions Department for review.

Once all required documents are in the Admissions will review the child's case, present to our physician for review and verify benefits. You should hear from Admissions within 24hrs Mon- Fri of receipt of all documents and if we feel that Hillside is the right fit to work with you and your child.
