Community Intervention Program
Hillside's Community Intervention Program provides intensive family-based treatment services to support families in crisis. Services are geared toward:
Ø
Preventing unnecessary out-of-home placement of children
Ø
Facilitating reunification of children who have been in out-of-home placement.
OUR SERVICES
v
Assessment of the client and their family with emphasis on strengths, interests and abilities. Referrals for special assessments are made as appropriate.
v
Flexibility in service delivery - depending on needs, either a standardized package or tailored services are available.
v
Crisis intervention to ensure that immediate treatment is received, factors leading to the crisis are identified, and new skills to avert future crises are taught.
v
Skill development - teaching parenting, behavioral management, conflict resolution, anger management, etc.
v
Dialectical Behavior Therapy (DBT) is provided to youth who engage in impulsive, aggressive, self-injuring or suicidal behavior. The
youth and their therapeutic foster family are assigned to a DBT coach and they
learn new skills while participating in the DBT skills group.
v
Staff availability 24 hours a day.
v
In-home therapy.
v
Case management to ensure that appointments are kept, treatment plans are adhered to and necessary services are obtained.
v
Department of Juvenile Justice (DJJ) community monitoring for youth with a mental health diagnosis.
v
Identification and development of appropriate community resources, advocacy, and assisting the client and family to access needed services.
v
Contracts with Department of Family and Children Services (DFCS), Promoting Safe and Stable Families, and
Dekalb County Youth Matters Program.
OUR GOALS
v
Help the child make a successful transition to a less restrictive environment.
v
Maintain the safety of children in the home.
v
Observe the client in the natural home setting so behaviors can be identified and addressed.
v
Include the whole family in determining treatment goals and evaluating results.
v
Identify the strengths of the client and family.
v
Intervene early and support the family to avert crisis.
v
Work with the family to develop new parenting skills and coping mechanisms.
v
Link the client and family with resources in their community.
v
Foster eventual independence and self-sufficiency.
v
Lower the cost of treatment for troubled children by avoiding more costly and disruptive out-of-home placements.
